What is actually included?

Select what each written offer says. “Not provided” means you still need an answer. Use “Marked not applicable” only when the offer or dentist says so. Enter each fee once; use “Included in another listed fee” for bundled items.

Fees are optional. Use numbers such as 1250.50, without commas. Enter 0 only when a zero charge is explicitly stated.
